Earn Your Wellness Credit
OSU partners with Catapult to conduct preventative biometric screenings for OSU employees and their covered spouses enrolled in one of the OSU BlueCross BlueShield health plans. Catapult conducts free, confidential health screenings on campus for your convenience. The screening consists of a diagnostic blood test, measured biometrics, a printed personal health report and a consultation with a nurse practitioner.
Employees who complete the Catapult screening will receive a $30/month health plan premium credit.

- I'm a new employee. When can I get my screening?
Screenings are only available to those enrolled in one of OSU's health plans. Following your hire, please wait until your benefits have taken effect to schedule your appointment. Catapult will not see you for your appointment if scheduled prior to your benefits taking effect.

- Do I have to be seen at the same time each year?
No. You may schedule your visit at any time. For appointments completed from Jan. 1 through June 30, the credit will apply through the remaining calendar year. For appointments completed from July 1 through Dec. 31, the credit will apply through the remaining calendar year and the entire following calendar year.
- What should I bring to my appointment?
Bring your Blue Cross Blue Shield card to your appointment.
- How does the wellness credit work?
OSU offers a $30 premium credit for completion of the Catapult screening. A screening is considered complete once the employee has participated in both the biometric screening and the nurse practitioner consultation.
For appointments completed from Jan. 1 through June 30, the credit will apply through the remaining calendar year. For appointments completed from July 1 through Dec. 31, the credit will apply through the remaining calendar year and the entire following calendar year.
- When can I expect to see the wellness credit applied to my premium?
The wellness credit in the amount of $30 will be applied to your premium the first of the month following your appointment and consultation with a nurse practitioner. For example, if you conduct your screening on Oct. 29 but don't have your call with a nurse practitioner until Nov. 1, the premium credit will not apply until Dec. 1.
- What if I am not eligible for a Catapult screening?
If you were not able to receive a Catapult Health Preventive Checkup this year because you were pregnant or delivered within the last 60 days, you may have your OB/GYN complete this form.
- If I regularly see my primary care provider, can I still get a Catapult check-up?
Absolutely! Blue Cross Blue Shield will cover both preventative screenings.
