Form 1095-C's for 2021

Form 1095-Cs for 2021 are now available to employees through Self-Service Banner. OSU Human Resources is in the process of printing and mailing forms for those that did not consent to receive an electronic form. OSU is required to send Form 1095-C for the 2021 year on or before March 2, 2022. The form serves as a "proof of healthcare insurance statement' that shows the IRS OSU employees have been properly covered. According to the IRS website, while the information on these forms may assist in preparing a tax return, they are not required.
You can view your 2021 Form 1095-C by following these instructions:
- Log in to Self-Service Banner
- (Choose Self-Service Under Applications)
- Select Employee
- Select Tax Forms
- Select 1095 Employer-Provided Health Insurance Offer and Coverage Statement
- Select 2021 Tax Year
If you wish to print the form, click on the “Printable 1095-C” button at the bottom left of the page.
