Spouse & Child Supplemental Life Insurance Coverage

Employees can purchase Supplemental Life insurance on their spouse and dependent children.  At any point during the year, employees can request to add supplemental life insurance or increase current insurance by completing and submitting a Life Insurance Evidence of Insurability (EOI) form.  The EOI can be found at

Employees can elect spouse supplemental life insurance coverage up to 50% of the total amount the employee has in life insurance on their self (basic and supplemental combined).  Within 30-days of an employee’s hire date, an employee can elect up to one-time his/her annualized salary not to exceed $125,000 in spouse supplemental life insurance.  During annual enrollment, employees who have spouse supplemental life insurance have the option to increase their spouse supplemental life insurance by $5,000 without EOI.

Employees have four choices of coverage for child (ren) supplemental life of $2,500, $5,000, $7,500, or $10,000.  All eligible children will be covered.  Children are eligible until age 21 regardless of school enrollment.  Children can be covered beyond age 21 if a full-time student.
