Benefits Education - Dependent Verification Documentation

Annual Enrollment is here!  During annual enrollment you can elect to add and drop dependents from your benefit plans.  When you login to Web for Employees, http://webemp.okstate.edu/ and make the election to add dependents, an “Add/Drop Spouse and/or Dependent(s)” form will populate.  You must complete and submit this form to OSU Benefits to have dependents added to your health, dental and/or vision insurance.  Along with the form, you also need to submit documentation that verifies your dependents.  Acceptable documentation includes:

  • Marriage license
  • Birth Certificate
  • Legal documents (custody agreements, adoption, guardianships)
  • Recent federal tax return (please only submit the page that lists your dependents and you can black out income information)

The Affordable Care Act (ACA) requires employers to report all employee and dependent Social Security Numbers (SSN) of persons who are enrolled and covered under the OSU health plan.  Please make sure to include dependents SSN when completing and submitting the form during annual enrollment.
