Waiving OSU Health Insurance

Employees who waive OSU’s health insurance, must actively elect to waive insurance every year during Annual Benefits Enrollment.  Failure to complete the entire process could leave you with Health insurance or without the OSU provided FSA benefit.  The following steps outline the process:

  1. Between October 1 – 16, 2015, login to Web for Employees at
  2. Select “Process Annual Enrollment Form” from the Annual Enrollment tab
  3. Change your Health Insurance Plan from “BlueOptions” to “Waived/None”
  4. Click on the “Waiving Health Insurance for 2016” form.  Complete this form, attach a copy of your insurance card and submit to OSU Benefits at 106J Whitehurst, or, or fax to 405-744-8345
  5. Review all other benefits for any changes in Web for Employees
  6. Scroll to the bottom of the page in Web for Employees and click the “Submit” button
