Benefits Education - Long-Term Disability (LTD)

OSU offers long-term disability insurance provided through American Fidelity Assurance Co (AFA). “Long-term” refers to a disability lasting more than six months. This type of coverage is good to have regardless of age because you never know when you could be in a car accident or acquire an illness that would keep you from being able to work.

There are two coverage options available to employees:

-          50% Coverage

  • $50,000 AD&D Policy
  • Maximum monthly benefit of $6,000

-          60% Coverage

  • Maximum monthly benefit of $6,000

In addition, while on disability, you are able to keep your OSU insurance (health, dental and vision) for the first two years. Your premiums are billed to your Bursar account. If you are disabled before age 60, you can apply for a premium waiver of your life insurance. This means that you would be able to keep your life insurance with no cost to you.

If you do not currently have LTD insurance, but would like to apply for coverage, you can do so by completing a Proof of Insurability form and a Life Insurance/LTD Change form, then return it to the OSU Benefits office. AFA will then review your request for coverage and respond with their decision.

In the event that you need to file for Long-Term Disability or have any additional questions, please contact OSU Benefits at (405) 744-5449
